ELDERONE

An affiliate of Rochester Regional Health, a Program of All-Inclusive Care for the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement.As the areas only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.

RESPONSIBILITIES

• Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands
• Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
• Documentation. Accurately and completely document client care on activity sheet
• Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements

HS (Required)

LICENSES / CERTIFICATIONS:

CNA - Certified Nursing Assistant - New York State Education Department (NYSED), HHA - Home Health Aide certification - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
