Job Summary and QualificationsAs a Medication Safety Pharmacist, you will be responsible for the medication safety and quality activities at the facility. This includes the standardization of medication management practices, ensuring regulatory compliance, implementing national quality agenda related to medication management, development of medication safety programs and staff development/education. You will provide support for all medication management and quality initiatives at the facility and those impacted by centralized order entry.
You will work with multidisciplinary groups at the facility level to ensure key quality initiatives are in alignment with organizational priorities.
You will develop facility pharmacy quality plan in conjunction with division leadership and clinical pharmacy group to ensure adequate budgetary support for medication safety programs.
You will serve as an internal consultant on medication/patient safety and quality standards related to medication management (ie. TJC, CMS, NQSF, IHI and FDA standards) and ensure TJC ongoing readiness.
You will identify and facilitate medication safety improvement projects.
You will lead the facility medication safety committee, agenda items to include, but not limited to: ISMP quarterly review, policy review, tracers, survey results, MUE action items, RCA, FMEA, pump safety, and Med Watch and medication error review.
You will conduct gap analyses for specific performance improvement processes.
You will manage data reporting and information related to medication safety, including medication events and ADRs.
You will review and trend reported medication occurrences to identify and resolve potential system vulnerabilities.
What Qualifications you will need:
Texas Licensed Pharmacist
Pharm D or Masters degree in Pharmacy required
Graduate of an accredited residency program preferred
3 years experience as a clinician in a hospital setting or completion of Medication Safety Residency required
Experience in patient safety initiatives preferred
Presentation, research and publishing experience preferred
Required to maintain current pharmacist licensure
